入选标准

1) South Asians from India, Nepal, or Pakistan; 2) have a smartphone and can communicate in English, Hindi, Urdu, or Nepali; 3) are not receiving other interventions concerning self-management of health; 4) are willing and able to provide written informed consent in English, Hindi, Urdu, or Nepali’s versions.；

排除标准

Patients with diagnosed cognitive impairments will be excluded；
