Effectiveness of post hospital discharge support program to improve the well-being of people with dementia and family caregivers
The aims of this study are:- 1. To investigate the efficacy of (1) a mixed mode intervention of respite and day care service and (2) a day care active rehabilitation intervention on improving the functional condition of people with dementia discharged from hospitals and reducing caregiving burden; and 2. To investigate the efficacy of a brief consultation meeting on enhancing dementia caregiving competence.

People with clinical diagnosis of dementia, unplanned hospital admission, and medically fit for discharge. He / she would be medically stable without infection risk to others.；
请登录查看People with significant psychiatric problems which may harm himself / herself or other care recipients, in regular need for complicated or risky medical procedures e.g. dialysis, airway suction, tracheostomy which JCCPA may not be able to accommodate with current and proposed capacity, care recipients whose family caregivers have no intention to take care of them at home, old age home residence for more than three months, those who are not able to travel to JCCPA.；
