试验目的

This project aims to evaluate the effectiveness of the Pilot Scheme in providing transitional care and support services, and facilitating ageing in the place in the community. Moreover, this project will identify key elements that will contribute to a sustainable service model and make recommendations or improvement measures for providing transitional care and support services for elderly persons discharged from public hospitals after treatment.

Elderly and family caregivers: 1) persons who are eligible for the Pilot Scheme; 2) persons who are able to provide written consent. HA professionals, service providers, care managers 1) persons who are eligible to provide services under the Pilot Scheme; 2) persons who are able to provide written consent.；
