Community eHealth Care Project
The project aims to develop and evaluate the nursing support protocol for effective self-management of chronic conditions and to promote healthy ageing in the community. The objectives are: To provide intervention guidelines and trainings to healthcare professionals to build up community-based professional capacity in supporting older adults with chronic conditions living in the community; To strengthen the older adults self-management knowledge and skills, carer support, and connect with the health service loop in the community; To evaluate the effectiveness of the programme, such as well-being, health behavioural change, health-related outcomes of (high blood pressure, diabetes, obesity/overweight, mild cognitive impairment), satisfaction with the care, health service utilization (A&E, hospitalization, specialist clinics, etc.), and others. To support the establishment of a health portal to support the elder centres services.
